A QR code is a two-dimensional barcode that stores information such as a link, a block of text, or contact details. Phone cameras and QR scanner apps read the pattern instantly, so nobody has to type anything in.
Frame generates static QR codes: the data is written directly into the pattern, with no third-party redirect involved. A dynamic code instead routes through a link you control, which is what lets it be edited or tracked later. Read the full comparison in our static vs dynamic guide.
A static QR code does not expire on its own. It keeps working for as long as whatever it points to stays valid, so a URL code stops working only if that page gets taken down or moved.
Some types do. Plain text and WiFi login codes are read directly from the pattern, so they work with no connection at all. A URL code still needs internet once scanned, since the phone has to load that page.

It can, if the logo covers too much of the code. That's what the scan-safety meter is for: it tells you when coverage is safely inside the error-correction margin before you download.
